KAHPERD Award Nomination Form

To nominate an individual for an award:

Make sure the person meets all the requirements for the award you intend to nominate him/her for.

Notify the person that you are nominating to confirm they want to go through the nomination process.  There is often paperwork that the nominee is required to complete.

The nominator should fill out the required nomination form (Kathy – can you link the nomination form to this statement?) and make sure that any other required, supplementary paperwork is sent by August 1, 2010, to Kathy Wagner, at the above address.

If the award requires an application process (i.e. NASPE Teachers of the Year, Adapted Teacher of the Year, Health Educator of the Year), send the paperwork to the nominated individual immediately so they can meet the August 1, 2010, deadline.  If an application is required, a web link is listed below the award description.

Check back with the person that you nominated to make sure they are filling out paperwork or providing letters of recommendation, if required.  Remind them of the August 1, 2010, deadline.  Materials must be received by this date to be considered for the award.
