Nomination Criteria:

• A current member of KAHPERD 
   
• Has conducted a Jump Rope for Heart event during the year prior to the year of nomination
   
• Award winners will be encouraged to present at the following year’s KAHPERD conference and submit an article for the KAHPERD journal/newsletter.

Nomination forms must be submitted by August 1st.

Candidate Requirements:

• Two letters of support/recommendation from an administrator, colleague, American Heart Association coordinator, student and/or parent
• A personal essay/profile summarizing professional history, service to the community(noting collaboration and support for event) , contribution to the profession (noting leadership in coordinating/implementing event and impact of the event), accomplishments (personal and professional) and/or other important information as it pertains to the award 
   
• Professional Headshot (JPEG, 300dpi minimum)

Candidate must submit requirements to the Awards Committee Chair by September 1st.
